Perforated Eardrum — Symptoms & When to See ENT

A “hole in the eardrum” (tympanic membrane perforation) often follows infection, trauma, or previous ear surgery. You may notice:

  • Reduced hearing or muffled sounds
  • Recurrent ear infections or discharge (otorrhea)
  • Ear fullness, popping, or intermittent pain
  • Whistling with nose blowing or water entry into the ear

What Is Tympanoplasty (Myringoplasty)?

Tympanoplasty (also called myringoplasty when only the eardrum is repaired) closes the perforation with a tissue graft and, when needed, reconstructs tiny middle-ear bones (ossiculoplasty). The goals are to protect the middle ear, reduce infections, and improve hearing where possible.

When Tympanoplasty is Combined with Mastoidectomy

In some patients with chronic ear infections or cholesteatoma, tympanoplasty is performed together with a mastoidectomy. This combined approach, called tympanomastoidectomy, removes diseased tissue from the mastoid bone while repairing the eardrum — helping to restore hearing and prevent recurrence.

How the Procedure Works

Your ENT surgeon evaluates the ear canal, eardrum, and middle ear—often with hearing tests and, if indicated, imaging. On surgery day:

  • Day surgery under local or general anesthesia
  • Microscopic or endoscopic ear approach
  • Graft placement (e.g., fascia or cartilage) to close the perforation
  • If needed: ossiculoplasty to improve sound conduction

Recovery & Aftercare

Most people return to desk work within a few days. Hearing improves gradually as the ear heals.

  • Keep the ear dry; avoid swimming until cleared
  • Do not blow your nose forcefully during early healing
  • Avoid heavy lifting and straining for ~2 weeks
  • Follow-up visits to check graft take and hearing
